  1. Make an online donation today. We’ll send you an email receipt right away.
  2. Check with your firm’s human resources/personnel office.
  3. Fill out the form they give you. The FDNY Foundation Tax Identification Number is 11-2632404. This may also be called a Employee Identification Number or EIN on the form.
  4. Mail the form and a copy of your receipt to FDNY Foundation, Matching Gift Dept., 9 Metrotech Center, Room 5E-10, Brooklyn, NY 11201.
